Electronic Versions of Forms


You've come to the right place to download forms for electronic submission

Click this link to download the .pdf files you want. However, you might want to take a moment to read the next two sections first.  We do not control how you have set up your computer's browsers. Some browsers will automatically download a file when you left click on it.  Other browsers are set to open the file instead.  

The next two sections explain how to download a file if you are using a computer that runs Microsoft Windows™.

    Recommended way to download

    The best way to download a file is to right click and click and then select "Save link as"

    If a form opens up in your browser--don't fill it out. Save it to your computer.

     If you left-clicked on a link and you see a form that looks like one of these...

     ...that means that the form has opened in your browser. Save the form to your computer by pressing <ALT>+F to bring up the "Save As Dialog box." Then, close your browser and fill out the form.

      Consider renaming the file to something that you'll remember

      If you rename the file so that it includes your last name, first name, the words "Public Record Request" or "Notification Request" and a date, it will be easy for you to search for it on your computer.

        I need another form, but can't find it

        As a courtesy, the Parole Board places forms that are developed by other agencies on our website. We have made electronic versions of all of the forms that are our responsibility. If you are looking for an electronic version of a form that does not appear here, please visit the website of the Division of Parole and Probation (P&P).

        Why you may need to contact P&P

        The Parole Board is a body that makes parole decisions.  P&P is an enforcement division that employs sworn officers. As a courtesy to those who visit our site, we provide forms such as the Appeal of Lifetime Supervision Conditions and its instructions; Parolee Statement Of Financial Hardship; and the Lifetime Supervision Release Petition Packet-2014.  P&P has not yet provided us with the electronic versions of the forms that are part of their workflow.
